Organisation Level Licensing - This form is used to apply to amend:
- The nominee for the licence
- The organisation named on the licence
- The licence function
- The care services or service delivery sites listed on the licence

The guidelines are also relevant to carers who are subsequently granted long term guardianship of the child under the Child Protection Act 1999.
